Are You a Patient of CH - Laconia or CH - Franklin?

As of August 1, 2022, My Patient Connect launched for Concord Hospital - Laconia and Concord Hospital - Franklin patients. If you used the previous patient portal associated with Concord Hospital - Laconia and Concord Hospital - Franklin, you still have access to your portal account, however, messaging is no longer available. You need to self-enroll in My Patient Connect to send messages to your providers' offices.

Proxy access is the official name for gaining access to another person's account. Parents may request proxy access to their minor child's health information. Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course.
